Create and save patient records for yourself and your family and easily generate Medication Risk Maps

Save all your medications, herbs, vitamins, and supplements including the dates you started and stopped each drug and the condition you are taking each medication for. Enter any untreated conditions, some of which may interact with your medications.

Group meds by medication status

    • Active Meds - medications you commonly take - whether every day or only when you need to, such as when you have a headache or a urinary tract infection
    • Proposed Meds - medications you or your doctor are considering. This lets you see how they will interact and combine with your other medications, even before you start taking them.
    • Inactive Meds - maintains a history of medications you have taken so you can see what impact they may have had or easily record when you start taking them again by adding them to your Active meds

Enter the conditions you take your meds for and any untreated conditions such as pregnancy and your age and gender

This additional information allows your Medication Risk Maps to show you more specific risks including medications that are contraindicated (should not be used) for people with your conditions.

No matter what their age, anyone who is taking prescription drugs or over the counter medications is likely to experience side effects and combined risks. If the meds are for Grandma or for the newest member of the family, it is critically important to know what side effects to look for and to be ready to tell your doctor when you suspect theme.

Take your medications list with you

Make sure you have a list of your medications when you go to the doctor or in any emergency. You can print your list from SURVEYOR Health or use one of our partners such as Polka to maintain a list on your iPhone.

If your online personal health records are kept with one of our partners, you can import your meds from them

Medication Risk Maps works with Microsoft HealthVault

Currently SURVEYOR Health works with Microsoft HealthVault. HealthVault allows you to download your medications from your pharmacy into your HealthVault medications list. When you provide permissions, SURVEYOR Health can import those medications directly into a SURVEYOR Health patient Med Cabinet. This can save you the effort of entering the medications yourself.

Create and save reference records

When you want to do some research without changing a patient record, you can create and save med lists that are not attached to any specific patient. This allows you to build your own reference library of medications you commonly consider or want to check out.

You can create and save multiple reference records and med lists so that it is easy to go back to them when you need to.

Combine Medications

You can create and save a reference record that shows the combined risk from a group of medications, just like in a patient record, without attaching it to a specific person.

Compare Medications

You can create and save a reference record that compares the risk from several different medications. For instance, if you are researching pain medications, you may want to compare the side effects of aspirin, acetaminophen, and narcotics.

What if i don't become a member?

You can still create basic Medication Risk Maps by entering your medications each time you visit SURVEYOR Health.
