Introduction
The Service is an online site that enables you to create personalized Medication Risk Maps you can use to identify, diagnose, and resolve adverse drug side effects and interactions from your combination of drugs, herbs, vitamins and minerals.
When signed into a member account you can choose to share information from separate programs and systems that the Service can connect with ("Partner Applications") to create the personalized Medication Risk Maps.

Collection of your Personal Information
To visit the SURVEYOR Health web site, you do not need to enter personal information.
To join as a member and register a member account, we require you provide your e-mail address and user name, create a password and create an initial patient record. We will use this e-mail address to send you an e-mail confirming your membership creation, and to send you notifications, such as e-mail notification when new SURVEYOR Health services and features become available.
To store and access your Medication Risk Maps with SURVEYOR Health in your member account, you will need to enter some of your personal health information. If you create Medication Risk Maps for other family members, you will need to enter some of their personal health information.
Personal information collected on the SURVEYOR Health web site is stored and processed in the United States of America (USA).
Use of your Personal Information
We do not use or disclose your information except as described in this privacy statement. We use personal information collected through our services, including health information such as medications, to provide the service you have registered for, and as described in this privacy statement.
We use your non-personally identifiable information to track and monitor aggregate usage of our web site, for internal analysis, quality control, and improvements to our service. We collect such information by using cookies, web server logs and other standard web technologies.
In support of these uses, SURVEYOR Health may use personal information to provide you with important information about the Service, including critical updates and notifications.

Partner Programs
A key value of the SURVEYOR Health web site and services is the ability to share your health information from Partner Programs to create personalized Medication Risk Maps.
A member account allows you to share health information from Partner Programs with the Service. You decide which Partner Programs you want to use. To use Partner Programs you must approve (or deny) the Service’s access. The access request will include (a) the type of information the Service will access and (b) what the Service wants to do with the information (view, add, modify).
A user of the Partner Program who has the appropriate level of access must affirmatively authorize the Service’s access to any health record in the Partner Program account. SURVEYOR Health will not disclose your data without express consent. You can freely grant and revoke the Service’s access to the records stored in Partner Programs. The access you grant to the Service and to the Partner Programs is valid until you revoke that access.
Partners may have different Privacy and Security guidelines so be sure to read their information for a full understanding of their service.
Currently, SURVEYOR Health only requests permission to view the medication data portion of your health records from Partner Programs in order to create personalized Medication Risk Maps for you. The lists of medications accessed from Partner Programs are available to you in the online session during which they are accessed, may be saved in the appropriate patient record, and are associated with your member account in our server logs.
Research Partners
SURVEYOR Health participates in a variety of medical research initiatives and one of our goals is to contribute to scientific and medical research. To help achieve these goals, SURVEYOR Health may enter into partnerships with research, commercial and academic based organizations that conduct scientific and/or medical research. Our collaboration with such organizations may include access to our databases of medical information without knowing the identities of the individuals or their personal account information. As a part of our participation in these initiatives, we require that the partnering organizations agree to maintain confidentiality consistent with our Privacy Statement and Terms of Use. SURVEYOR Health does not sell, rent or lease any of its member account information with these partner organizations.
Commercial Partners
SURVEYOR Health may enter into commercial arrangements to help enable partners to provide our service to their customers and/or to provide you with access to their products and services. We may collect fees for these referrals. Prior to providing any personal information to these partners, we will request your informed consent.

We also collect information for your initial patient record We may request other optional information, but we clearly indicate that such information is optional. You can review and update your account information at any time when you are online and on our web site. You can modify, add, or delete any optional account information by signing into your SURVEYOR Health account and editing your account profile.
You can close your account at any time by signing into your SURVEYOR Health account and editing your member account. We wait ninety (90) days before permanently deleting your account information in order to help avoid accidental or malicious removal of your health information.
How we Aggregate Iinformation and Statistics
SURVEYOR Health may use aggregated information from our services to help improve the quality of the Service and for marketing of our services (for example, to tell potential advertisers how many member accounts are registered on our Services).
This aggregated information is not associated with any individual member account.
Communication Preferences
To keep you informed of the latest new products and services from SURVEYOR Health, we will send you an e-mail notification. If you do not want to receive the notification, you can uncheck the box that requests the notification when you create your member account for the Service, uncheck the box in your My Account page at any time, or unsubscribe through a link at the bottom of the notification. If you later decide that you want to receive the notifications, you can request it by checking the box on the My Account page.

SURVEYOR Health is committed to protecting the security of your personal information.
